Annual Chapter Report Form

Important
This report is the basis for which the annual Outstanding Chapter Awards are granted and listings in the SPS Information Handbook are compiled. In order for SPS Advisors to be considered for the Outstanding Chapter Advisor Award ($5,000), this report must be filled out in full and submitted by June 15.

Please copy, paste & save --or-- print the confirmation pages that appear after completing Step I and Step II of your report for your files. If you would rather send a paper report via US mail or fax, please use our PDF or Microsoft Word© version, or use the forms found on pages 24-28 of your SPS Information Handbook.

FOR THIS ACADEMIC YEAR, DESCRIBE YOUR CHAPTER'S:


1. Participation in SPS Zone meetings.

Include titles and authors of papers presented by your chapter members, attendance by your members, distances traveled, tours, etc. Include meetings attended on any Zone.

2. Participation in other professional meetings.
(e.g., SPS members who presented papers at AIP Member Society meetings, etc.)

3. Involvement in SPS programs (include applications made by individuals in your chapter).
These include the Marsh White Award (proposal submitted? Funded?), Blake Lilly Award, Undergraduate Research Award, ICPS Award, nominations for Outstanding Chapter Advisor, chapter members nominated for Associate Zone Councilor, members who applied for scholarships, etc.

4. Participation in local SPS chapter meetings.
List guest speakers, films, business meetings, tours, student presentation rehearsals, etc.

5. Participation in local SPS-sponsored social events.
(e.g., picnics, Sigma Pi Sigma banquets, floats in parades, etc.)

6. Involvement in outreach efforts to the grades K-12 or the general public.
Please include projects such as “Physics Circuses,” classroom demonstrations, tutoring services, assistance with school science fairs, open houses, promoting the US Physics Olympiad Team, etc.

7. Participation in community service performed during the academic year.
Include chapter-organized charitable service, fundraising, etc.; performed for individuals, families, charities, non-profit groups, etc.

8. Contributions to student recruitment and retention in your department.

9. Interactions with your department’s alumni.
Include newsletters, invitations to speak, arranging internships with local industries through alumni contacts, etc.

10. Did you host a Zone meeting this year? Have you committed to host a Zone meeting any time soon?
If your chapter hosted a Zone meeting during 2002-03, describe the program, institutions represented, number of papers presented, guest speakers invited, tours conducted, etc. Please attach a copy of the program and photographs, if not already submitted for publication in the SPS Observer.

11. Strategy for promoting SPS membership.

13. Describe events that the SPS Chapter co-sponsored with other campus groups or participated in with other campus organizations.
(e.g., Co-sponsoring a colloquium with the mathematics club, organized participation in a campus blood drive, etc.)

14. Describe events that your chapter held jointly with other SPS chapters.

15. Describe other projects and functions not already covered in this report.

16. Describe the over-all health of your chapter.

17. Describe chapter problems. What can the national office do to better serve you?

20. Did your chapter host a Sigma Pi Sigma reception event or any other honor society events?
If your chapter hosted a Sigma Pi Sigma reception during the past year, describe the program, number of persons inducted, number of attendees, guest speakers invited, tours conducted, etc. Please submit a copy of the program and photographs, if not already submitted for publication in Radiations magazine.
21. Attachments
Attachments such as photos, programs, news clippings, etc., are welcome. Please e-mail any such attachments to sps@aip.org, along with your name, chapter, and chapter number, if feasible. For larger files (images, movies, etc.) it is probably better to send us an appropriate URL in question #19 above, by email, or on a disc by snail mail.
